While you can use our secure Patient Portal to request appointments, our portal has a number of convenient features. You can update personal information, view your medical history and statements, and make online payments.

NEW PATIENT CHECKLIST
A link to complete Patient Information Forms will be emailed to you 24 – 48 hours prior to your appointment. If you don’t receive these forms, please call (402) 496-0404.
You will need:
- Photo identification
- Current insurance card
- List of current medications
- List of allergies
- Medical history
- Referring physician’s notes, if applicable
- Past MRI, CT, or X-ray scans or nerve conduction studies, if applicable
- Copay or required down payment
- Auto accident liability form, if applicable
- Workers' compensation form, if applicable
